Abstract

Frenal attachments are thin folds of mucous membrane with enclosed muscle fibers that attach the lips to the alveolar mucosa and underlying periosteum. The frena may jeopardize the gingival health when they are attached too closely to the gingival margin, either due to an interference in the plaque control or due to a muscle pull. In addition to this, the maxillary frenum may present aesthetic problem as diastema in incisivus central maxilla. One of treatment for this case is frenectomy. Frenectomy is the complete removal of the frenum, including its attachment to the underlying bone. This procedure can be accomplished by classical technique with scalpel and blade for surgery. Before surgery, patient systemic condition is one of important factor that the physcian must evaluate carefully. Diabetes mellitus is a group of metabolic diseases charaterized by chronic hyperglicemia resulting from defects in insulin secretion, insulin action or both. Insulin resistance is mainly responsible for type 2 diabetes. Type 2 diabetes is caused by bad lifestyle as excess sugar intake and its may determined by gene. Doing a surgery in high blood level patient can increased rates of surgical site infection. Because of that, regulation of blood level in diabetic patient is mandatory before doing a surgery. In this case report frenectomy was treated in patients with diabetes mellitus type 2 and controlled hypertension.
