Abstract

Introduction : Potassium has many beneficial effects on various cardiovascular and cerebrovascular diseases including stroke. Thrombotic process is closely asscociated with biology of platelet where potassium plays an important role in suppressing it?s activity. Additional treatment of potassium twice dose of daily intake for 3 consecutive days will decrease the activity of platelet in human. Additional potassium treatment is also said to decrease interleukine-1ß (IL-1ß) level which tremendously increase during cerebral ischemia. Aim of this study is to find the difference of clinical improvement on acute ischemic stroke patients receiving standard treatment and standard treatment plus potassium. Method : This was a clinical trial on acute thrombotic infarction stroke patients randomizedly allocated in control and treatment groups. The control group received standard treatment and the treatment group received standard treatment plus potassium (KCl). Assessment of neurological impairment according to the National Institute of Health Stroke Scale (NIHSS) performed on day 1, 4, and 7 of the study. Result : There were 23 subjects in control group and 24 subjects in treatment group. Median NIHSS score on day 1, 4, and 7 were 7, 6, and 4 in the control group and 7,5, 4,5, and 4 in the treatment group. The median NIHSS score improvement was statistically significant on day 4 (p = 0,022) and not significant on day 7 (p = 0,444). Discussion : There is significant difference of clinical outcome improvement on acute thrombotic infarction stroke patients with normal potassium level who are treated with standard treatment and additional treatment of potassium compared to those who do not get additional treatment.The difference is justified by NIHSS score on day IV of treatment. The difference does not appear on day VII of treatment.
