Abstract

The frenum is a mucous membrane fold that attaches the lip and the cheek to the alveolar mucosa, the gingival and the underlying periosteum. Maxillary labialis frenum may present aesthetic problems or hinder teeth movement in orthodontics care to manage central diastema. The management of such an aberrant frenum is accomplished by performing a frenectomy. Gingival enlargement also known as gingival overgrowth is defined as increase in the size of gingiva. Gingival enlargement can be caused by a wide variety of etiologies. The most widely employed surgical approaches for the treatment of gingival enlargements is gingivectomy. In this presentation a 19–year–old female patient referred to the Periodontics Clinic Dental Hospital Airlangga University by her orthodontic during the treatment with chief complaint high maxillary labial frenum and swelling of the gingiva in central insical. Clinical examination revealed the presence of a central distema caused by a high frenum attachment extending to the palatal interinsicisal region and localized gingival enlargement in central insical. After patient was explained the condition and the treatment, she was enrolled for phase I therapy. After phase I therapies were evaluated and next was to be continued to phase II therapy by scalpel surgical frenectomy and gingivectomy. Aberrant frenum and gingival enlargement correction by surgical scalpel of frenectomy and gingivectomy procedure is to reestablished a health gingival and aesthetic. Frenectomy and gingivectomy gives satisfactory result in treating an aberrant frenum and gingival enlargement.
