Abstract

Neurologic disorders are among the most common and devastating complications of HIV infection and AIDS. As patients with HIV infection survive longer in the era of HAART, it is likely that the prevalence of these disorders will increase. The purpose of this article is to enhanced our knowledge in diagnosis and treatment of peripheral neuropathies in Human Immunodeficiency Virus (HIV) patients. Peripheral neuropathies are common in HIV infection, and are associated with all stages of HIV infection. Symptomatic neuropathy occurs in approximately 10-15% of HIV infected patients overall. The synptoms of peripheral nervous system involvement vary with the stage of HIV infection and degree of immunsuppression. There are major clinical types of HIV-associated neuropathies, Seroconversion-realted neuropathies, Distal Sensory Polyneuropathy (DSP), acute and chronic inflammatory demyelination polyradiculomyelopathy and nucleoside associated toxic neuropathies. Distal sensory polyneuropathy occurs approximately 30% of patient with a CD4 cell count at below 200/mL. In order to provide effective treatment, it is essential to distingusih among the various forms of neuropathy and to correctly attribute them to primary HIV infection or to other causes, such as the neurotoxic effects of antiviral agents. Although peripheral neuropathy is generally not a life-threatening disease, the debilitating pain can severely impair the quality of life of affected individuals.
