Abstract
Antimikroba beta-laktam paling sering digunakan untuk pengobatan infeksi. Paparan antimikroba tersebut secara terus menerus ke beberapa strain beta-laktam akan menginduksi adanya mutasi gen penyandi enzim beta-laktamase pada bakteri tersebut. Mutasi tersebut menyebabkan aktivasi baru bakteri dalam melawan antibiotk beta-laktam. Enzim ini dikenal dengan spektrum β-lactamase. Infeksi yang disebabkan ESBL cenderung terjadi di negara yang dengan tingkat pendapatan rendah. Infeksi tersebut berdampak pada lamanya rawat inap jika pasien tersebut dirawat di rumah sakit dan memerlukan terapi antibiotik. Jenis penelitian ini cross sectional. Sampel dalam penelitian ini adalah swab rectal dari bayi usia neonatal dini dan pasca neonatal yang diambil dari 3 Puskesmas di wilayah Surabaya Utara mulai bulan Februari sampai April 2019. Besar sampel pada penelitian ini 200 sampel. Dengan teknik pengambilan sampel consecutive sampling. Variabel penelitian ini meliputi penggunaan antibiotik, komplikasi kehamilan, riwayat penggunaan alat-alat medis, hygiene sanitasi, diet, hygiene sanitasi, antibiotik, usia dan kejadian ESBL. Analisa data menggunakan Chi-square dan regresi logistik untuk multivariat Hasil penelitian ini ESBL yang ditemukan 51 dari 200 sampel dengan 42 gen ESBL yaitu CTX-M 21 (50%), SHV 2 (3,8%), TEM 1 (1,9%), CTX-M, TEM 7 (13,5%), CTX-M,SHV 6 (11,5%), CTX-M, TEM, SHV 5 (9,6%) Simpulan hasil penelitian ini adalah Terdapat perbedaan jumlah ESBL positif diantara usia neonatal dini dan pasca neonatal dari 3 Puskesmas di wilayah Surabaya Utara, prevalensi ESBL pada usia pasca neonatal yaitu lebih besar dibandingkan usia neonatal dini
