Abstract

Backgrounds. High prevalence of multidrug use combined with age-related alteration of pharmacokinetic and pharmacodynamics makes elderly hypertensive patients vulnerable to drug related problems (DRPs). The objective of this study was to identify the usage pattern and both actual and potential DRPs of anti-hypertension therapy in geriatric patients. Method. A prospective observational study was conducted in Geriatric Ambulatory Station, Dr. Soetomo Hospital – Surabaya from March to May 2012. The demographic data of hypertensive patients was collected by reviewing the patient’s medical record. Analysis of drug side effect was done by interviewing the patients and others DRP by reviewing the drugs and patients condition, and literature reviews Result. From 350 samples showed that hypertension was most commonly found in “young old” group (66-74 years old) (50.9%). More than half of the patients had diabetes mellitus as comorbid. More than 60% of the patients received more than one anti-hypertension therapy. ARB, CCB, and ACEI were three most frequent used drugs. Actual and potential DRPs were identified in 3.2% and 62% cases: inappropriate drug of choice (1.4%), inappropriate dose and frequency (0.3%), side effects (2.1%), and potential non-benefited drug interaction (62.0%), whereas one patient could have more than one problem. More than two third of the patients could not achieve the target blood pressure according the JNC VII and AHA recommendations. Conclusion. Management of hypertension in geriatric patients still left unresolved preventable DRPs.
