Abstract

Morphine is a strong opiate analgesic recommended as first-line treatment and gold standard to relieve moderate to severe pain or cancer-related pain which is untreatable with mild analgesic. This study was aimed to examine the use of morphine at Palliative and Pain Free Installation in Dr. Soetomo Teaching Hospital Surabaya. This study looked at the daily dose and frequency, the effect of morphine and other analgesics on patient’s quality of life, and actual drug related problems. Stage III or IV breast cancer patient were included in this study. The retrospective study was done between March 1st – November 1st 2014. Questionnaire, followed by interview were carried out to explore the use of morphine for their pain. Medical records of all patient were looked up for additional data. Sampling was done by time limited sampling method. Results from this research showed that morphine dosing highly individualized to each patient. In addition, after administration morphine, patient’s quality of life is increase. This is evidenced by : morphine decreasing the intensity of pain experienced by patient, improving patient’s ability to move, increasing patient’s quality of sleep and decreasing patient dependency level with others. Actual drug related problems that identified were breakthrough pain (80%), noncompliance (40%), ineffective dose (20%), sedation (100%), constipation (40%), nausea and vomiting (20%),and vertigo (10%).
