Abstract

The high prevalence and incidence of complications due to diabetes mellitus impose a significant economic burden both globally and in Indonesia. The purpose of this study was to obtain an overview of the total cost of diabetes mellitus from inpatient care to outpatient visits, as well as to see the factors that affect direct medical costs. This study were conducted based on the perspective of health care providers, using a cross sectional study design with retrospective data collection. The study sample are all patients (population) who were hospitalized during July - December 2019 and met the inclusion criteria. The total of 171 patients analyzed, 94 patients (51.5%) were diabetes mellitus patients without chronic complications based on the charlson comorbidity index consisting of hyperosmolar complications (E11.0), complications of ketoacidosis (E11.1), specific complications (E11.6). and nonspecific complications (E11.8). A total of 89 patients (52.0%) were women. A total of 89 patients (52.0%) were women. The mean age was 59.73 years and length of stay of the patients was 7.35 days. A total of 74 patients (43.3%) used class II, 161 patients (94.2%) used insurance and 158 patients were using BPJS. 118 patients (69.0%) returned to the first outpatient control after being hospitalized. The cost of illness for diabetes mellitus patients with complications are Rp. Rp.2.654.047.597. Medical service fees, room fees and drug costs are the highest cost components of direct medical costs. The test results using multiple linear regression found that the factors that significantly affected direct medical costs were hospitalization, class of hospitalization and type of complications (charlson comorbidity index).
