Abstract

DMDD is a new diagnosis with a set of characteristic symptoms of severe irritability, setting, and chronic and adolescents. Children with DMDD showed a low frustation tolerance and experiencing difficulty in regulating emotions, low tolerance in a state of distress and trouble in controlling behaviors. Between attack, children showed persistent irritability mood throughthout the day which can be observed by parents, teachers and peers. Diagnosis is made when this disorder lasts in at least two settings in different places (home, school, neighborhood). Onset of symptoms before age 10 years, but diagnose can't be made before the age of 6 years and above 18 years. Symptoms that stick out are chronic irritability, known by Leibenluft as severe mood dysregulation (SMD), which in the DSM V known as DMDD. Chronicity of symptoms indicating a mismatch with symptoms episode according to DSM IV, where manic episode lasts at least 4 days and hypomania for at least one week. Based on the results of researches, DMDD will evolve into anxiety and depression disorders, and not a bipolar disorder in adulthood. SMD can be distinguished from DmDD by the onset of age and the presence of hyperarousal. In SMD, hyperarousal symptoms can be found on an episode of mania and sometimes on ADHD, chronic, and non episodic irritability with onset before 12 years of age. There has been no appropriate tools to assess and diagnose DMDD. Child Behavior Checklist Dysregulation Profile (CBCL-DP) can be used as a tool to identify patients with severe mood disorders. High Scoring indicate persistence problems with bad outcomes such as substance abuse, suicidality, and functional disorders in facing life.
