Abstract

Background: The focus of modern periodontal treatment are health and aesthetic. Gummy smile becomes one of periodontal aesthetic problem. The causes of gummy smile, including: delayed passive eruption, hyperactive upper lip muscle and vertical maxillary excess. Gummy smile be a good candidate for periodontal plastic surgery. Objective: to demonstrate the treatment of gummy smile with vestibuloplasty procedure. Case: A 24 years old female patient has excessive maxillary gingival display when smile. General condition is good, no systemic disease and ready to receive periodontal plastic surgery with vestibuloplasty procedure. Case management: First, labial infiltration anesthetic in maxillary anterior region. Partial thickness incision is made along the mucogingival junction from left to right canine. Parallel incision is made at labial mucosa approximately 10mm from the first incision. The two incisions are connected. Gingival epithelium is removed from the alveolar mucosa. Silk sutures 4.0 are placed to close the wound. Sutures is removed 21 days later. One month control after surgery, patient feel comfort and satisfied because her smile profile is normal. Conclusion: Vestibuloplasty is an alternative treatment that is effective to reduce gummy smile.
