Abstract

COVID-19 first emerged in December 2019, when a cluster of patients with pneumonia of unknown cause was recognized in Wuhan, China. COVID-19 is caused by the novel SARS-CoV-2. The disease is mild in most people, but in some (usually the elderly and those with comorbidities), it may progress to pneumonia, acute respiratory distress syndrome (ARDS) and multi organ dysfunction. IL-6 receptor inhibitor have been repurposed to treat COVID-19, but their exact role in treatment remains unclear. Tocilizumab (TCZ), a monoclonal antibody against IL‐6, emerged as an alternative treatment for COVID‐19 patients with a risk of cytokine storms recently. This study used an observational-retrospective method that observed the use of tocilizumab in COVID-19 patients. The subjects in this study were inpatients who were diagnosed COVID-19 and received tocilizumab therapy with or without comorbidities and infections. The number of patients who met the inclusion criteria in this study were 30 patients. The analyzed profiles included patient demographic data, clinical manifestations, related laboratory data as well as analyzing the use of tocilizumab. The data were taken using time limited sampling technique. The results showed there was no variation in the administration of Tocilizumab on 30 patients. Tocilizumab was given at a dose of 400 mg as a single dose by intravenous infusion over 1 hour. The use of tocilizumab as a therapy for COVID-19 patients did not show an improvement effect based on parameters such as leukocytes, CRP, NLR and ferritin which tended to increase the patient's mortality rate. IL-6 levels showed an increase after administration of tocilizumab. It suggested that tocilizumab was given at the early stage of patient’s severe or critical state of COVID-19. A further study regarding the effectiveness and the side effects using tocilizumab in COVID-19 patients is needed.
