Abstract

Discharge planning is a nurses learning process of patients hospitalized in the hospital. Discharge planning includes all treatments given to the patients from the time of admission , during hospitalization, until the time of discharge. It disciplinary assessment of patient/family needs and coordination of care, services and refferals. Discharge. Discharge planning in the ward was done to 6 patients with DHF and 8 patiens with GE, to evaluate the patiens compliance to the treatments that included oral and injection medicine, nutrition and activities. Discharge planning implemented and simply given as more information. Observation that was done included the laboratory of thrombocyte count in DHF and leucocyte count, BUN , SC, Na and K measurements in pre and post test for patients with GE, and also ALOS of each patients with GE and DHF The objective of this study was to identify the presence of the effect of discharge planning on medicine, nutrition and activities patients at woman tropical ward, Dr. Soetomo hospital, Surabaya. This study used quasy-experimental design. The population comprised 14 respondens, consisting of 7 respondens as tretment group, and other 7 respondens as control group, recruited using purposive sampling. Data were controlled using questionaire, comprising demographic data, medicine, nutrition and actvity. Data were analyzed by using Wilcoxon Signed Ranks, Mann Whitney Test and T Test with significance level of p ≤ 0.05. The result of this experiment showed that the effect of discharge planning to the complience of therapy in DHF which is p = 0.025 in treatment and p = 0.028 in control . For GE the value of p = 0.027. This meant H1 was accepted implying that discharge planning have influenced the patiens compliance to therapy. But if the medicine, nutrition and activities were separated, the result was insignificant. And this meant the components of therapy could not be separated and should be comprehensive. Further study should be measured to the effect of discharge planning on patient safety in patients with tropical disease.
