Abstract

Background: Mesothelial cyst is a very rare tumor. It poses a challenging preoperative diagnostic challenge due to its nonspecific clinical presentation. Case description: We report a case of a 31-year-old woman complaining of a palpable mass from the lower part of the abdomen. Our initial diagnosis from the clinical presentation and radiology features was a suspected cystic mass with ovarian origin. We performed a laparotomy; during surgery, a cystic mass was found, which originated from the fundal part of the uterus. Histology examination revealed the mass has a benign nature and characteristics like a mesothelial cyst. Outcome: Resection of the cystic mass was performed. Postoperative histopathology examination shows a benign mesothelial cyst originating from the uterus. Regular follow-up until 1-year postoperative procedure shows no sign of recurrence. Conclusion: This case emphasizes the importance of considering mesothelial cysts in the differential diagnosis of cystic masses in the pelvic region. Operative procedure followed by histopathology examination revealed a mesothelial cyst of the uterus, despite the initial suspicion of an ovarian origin based on imaging studies. The successful management involved surgical resection of the mass and regular follow-up to monitor potential recurrence. © The Author(s). 2026 Open Access.
