Evaluation based on surveilance attribute about DHF control program has been done specifically evaluation for case surveilance, vector surveilance and vector control. This research variables were simplicity, flexibility, acceptability, sensitivity, predictive value positive, representativeness, timeless, data quality and data stability From this research, discovered that the activity of surveilance in the village of Putat Jaya reviewed from disease contol program management was not succeed into decrease incidence rate of DHF and increase free number larvae (ABJ). It could be seen from Incidence Rate in 2013 has reached 133/100.000 with ABJ Percentage only 84,01%.
