Abstract
The purpose of this research is for evaluating the occupational accidents and the factors that related to the accident at PT. United Tractors Semen Gresik between 2010-2014. This research was done by using observational method, interview method, and secundary information resources. The sample of this research are all workers who have had occupational accident between 2010-2014. The data is collected by interviewing the workers, OSH stake holders, field observation, and from the accident register book. The result of this research are known that most of occupational accident happened among workers in 36-40 group ages for 11-15 years of working periode. The accident happened mostly at 2014 at limestone in transport vehicle unit.the accidents happened mostly in sunny day at the first shift. The occupational accidents of PT. United Tractirs Semen Gresik caused 112 work days lost from total 868.896 work hours per year among 341 workers. The results shows that the highest number of occupational accident at PT . United Tractors Semen Gresik occurred at 2014. The accident occurred 97 times in every 1,000 workers or 48 times the accident every one million hours of work in 431 workers. PT . United Tractors Semen Gresik 1,000,000 lost 12.8899,2 days of productive work days which caused losses to the company . The result of observation is PT. United Tractors Semen Gresik meets 50% criterias of 44 total criterias of occupational accident prevention program based on standard operational procedure. No increase in the means for prevention of occupational accidents in 2010-2014 ( Safe T -Score = 0.096391 ) .
