Abstract

Nursing documentation is written information about the status and progress of patient's condition as well as all nursing care activities carried out by nurses. model documentation is a frame of reference in reporting patient's condition that requires immediate attention or action.This study aimed to determine factors associated with the implementation of the SBAR model documentation according to the Theory of Planned Behavior in Operating Room, Floor V, Emergency Unit, Dr. Soetomo Hospital, Surabaya. Design used was analytic descriptive study using cross-sectional approach. The population in this study was all nurses in the Operating Room, Floor V, Emergency Unit, Dr. Soetomo Hospital, Surabaya. Sampling method was total sampling with inclusion criteria, and we obtained 37 respondents. The independent variables included attitude, subjective norms and behavioral control with questionnaire as instrument. The dependent variable was the implementation of SBAR model documentation with observation as instrument. Data analysis used Spearman Rho with the level of significance ρ ≤ 0.05. Results showed that attitude (p = 0.002), subjective norms (p = 0.01), behavioral control (p = 0.00) correlated with the implementation of SBAR model documentation. It can be concluded that attitude, subjective norms, and control behavior influence nurses to carry out documentation in accordance to SBAR model. The better the attitude, subjective norm, and control behavior of the nurses, the better the implementation of SBAR model documentation. Future studies need to observe the motivation and appreciation for nurses and leadership at unit level in the hospital to improve SBAR model documentation.
