Abstract

Background: The aging process results in physiological changes. That potentially increase health problems. Furthermore the decline of physical and cognitive abilities. There is a wide variety of factors that elderly people had risk for problems in medication management. Many of the risk factors related to inadequate medication management are items that are more prevalent in elderly people such as poor vision, grip strength, and cognitive decline. Aims: To reveal the profile of medication management for the elderly in a longterm care facility in Surabaya, including medication profile, medication storage, medication use, and medication disposal. Method: This research was a cross sectional descriptive study. Data was collected by interviewing respondents using a list of questions with directions to fill in the questionnaire. Before taking the real data, a pilot study was conducted. Result: Respondents were 38 elderly people and 10 caregivers. The most health problem experienced by respondents was cardiovascular disease related such as stroke and hypertension (18,2%). Almost half of respondents had one health problem and consumed one medicine everyday. Almost all respondents obtained medication from pharmacies through the physician prescriptions. Among all of the drugs used, there were 81% evidence of proper medication use according to the goals of therapy, time of use, and dosages. There were seven of potential drug interaction found on the four elderly. The medicine was managed by caregivers in a room which had good conditions for used storage. All the unused medicine were kept medicine until the expiration date. The caregivers did not destroy medicines before their disposal. Conclusion: Medication management in UPTD Griya Werdha Surabaya can still be optimized. Pharmacist potentially contribute in medication management of the elderly, as a team with other health care members
