Abstract

Background. A big bladder stone often associated with bladder malignancy. Based on the tumor immunology theory, bladder malignancies in association of bladder stone ensued due to mechanical irritation from the stone or chemical irritation from the stone substances. Until recently there were no data on early predictor factors that related to the molecular changing mechanism of bladder mucosa induce by calcium-oxlate bladder stone. Objective. To demonstrate that molecular changing of the bladder mucosa induced by calcium-oxalate bladder stone as the mechanism in the developing bladder mucosal malignancy. Material and Method. Thirty male Wistar strain of Rattus Norvegicus (RN) were divided into 3 into 3 experimental arms; control group, vesicostomy group, and intravesical calcium oxalate stone group. After 4 weeks period of study, each group underwent a series of tests to determine the serum level of IFN- using ELISA; the number of cell that express the mucosal IFN- , mutant p53, mutant pRb, mutant pRas, mutant pBcl-2 using immunohistochemistry techniques. On the collected data, we performed analysis of distribution normality using the Kolmogorov-Smirnov test followed by data analysis using the One Way ANOVA test. For the ranked data, analysis were performed using Kruskal-Wallis test. To measure the correlation of the collected data, we performed the Spearman’s rho test. Results. There were significant differences between the quantities of cells that express IFN- on the bladder mucosa (p=0,0001), cells that express mutant pRas (p=0,005), and cells that express mutant pBcl-2 (p=0,007). On the contrary, no significant differences in serum level of IFN- (p=0,217), expression of mutant p53 (p=0,593), and expression of mutant pRb (p=0,274). This study did not obtain significant correlation between cellular expression of IFN- on the bladder mucosa and the level of serum IFN- into 3 experimental groups. Conclusion. The present of calcium-oxalate stone for 4 weeks in the bladder induced molecular changing in the mucosal bladder as showed by increase of mutant pRas, mutant pBcl-2, bladder mucosal IFN- expressing cells.
