Abstract
Immunization is one of the promotive and preventive effort which increase the human immunity actively toward the diseases. WHO Regional Director for South- East Asia estimated that the immunization can save 2 up to 3 million people each year in all over the world. The accomplishment of UCI coverage at Surabaya City were not followed by the decreasing in cases of deseases that can be prevented by immunization (PD3I). The efforts to improve the quality of immunization can use the development of information system based on technology, one of which is the integration of information system and surveillance PD3I immunization program. The aim of this study was to develop a model of immunization and surveillance information system of PD3I at Surabaya Health Department. This research use the action research in the form of system development. Based on the result of research on the analysis of immunization surveillance information system problems and PD3I it said that there are system problems at the componet input, process and output. The information needed is the ammount of coverage complete basic immunization and booster, the number of PD3I cases in each sub-district, UCI sub-district, sub district with the cases of PD3I, vaccine quality, the priority area of immunization and the prevelance of PD3I who have not been immunized. The trial result showed that the surveillance information system of immunization and PD3I is quite easy and useful for immunization and surveillance officer of PD3I in Public Health center and in Surabaya Health department.
