Abstract

The chronic plantar ulcers of leprosy take a long time for healing due to the lack of growth factors and cytokines caused by autonomic nerve impairment. The standard wound care used routinely does not contain such components. The idea arose to apply the amniotic membrane stem cellsconditioned medium (AMSC-CM) containing a lot of growth factors and cytokines to improve wound healing of chronic plantar ulcer of leprosy. Purpose: To prove the effect of topical AMSC-CM on the healing process of chronic plantar ulcer of leprosy and comparing with framycetin gauze dressing (FGD). Methods: This is an analytic experimental research, comparing topical AMSCCM and FGD on healing process of chronic plantar ulcer of leprosy. Subjects have been collected through consecutive sampling and matching based on the occupation of patients and the size of ulcer, with sample size 11 chronic plantar ulcers each groups. Application of the drugs is every 3 days up to 2 months. Weekly measurement is the size of the ulcers, as well as monitoring the side effects and complications from both drugs. Data were analyzed analytically. Results: Ulcer healing in AMSC-CM group is better than FGD group, with significant clinical and statistical differences (p = 0.009 and p = 0.005). There were no side effects in both study groups. Complications such the increasing of the width of ulcer was found in one subject in the control group, but not directly caused by FGD and more caused by the claw foot condition. Conclusions: AMSC-CM could accelerate the healing process of chronic plantar ulcer of leprosy.
