Abstract

Periapical healing after endodontic treatment in periapical disease cases is the result expected by both patients and operators. Healing is essentially a tissue recovery from a tissue structure which is damaged by a disease back to its original structure so that the tissue can function as normal. To assess periapical healing the presence or absence of tissue recovery, required a certain criteria that can be used to indicate the progress of healing that can be achieved. Currently there are generally three approaches used to assess periapical healing, that approach: clinical, radiological, and histopatological. Assessment through clinical approach is achieved by patient history, clinical symptoms and examination of both extra-oral and intra-oral. Radiological assessment of the approach is the presence the size reduction of the lesions in the periapical healing process as typically seen in observations of local x-ray images. Basic assessment used by histological approach is the difference histopatological cell type or tissue structures examined in the healing process, under the microscopic observation. Assessment using histopatological approach is essentially using the morphological approach to the anatomy of an objective examination by looking at the condition of cells and tissues involved in the process of healing such as inflammation, capillary vasodilatation, exudation, proliferation of fibroblasts and the establishment of fibrous tissue. Developing alternative assessment methods of periapical healing is through imunopatologic approach. Assessment of healing using this approach based on the concept of morphofunctional. Basic principle of morfofunctional concept, is the immunologic empirifcation integrated component that can show imunopatologic changes that occur in the process of healing, and subsequently the changes is interpretated scientifilcally by the use of logical imunopatologic reasoning. Assessment of healing approach is expected to reveal changes imunopatologic morfofungsional more accurate than anatomic morphological changes such as the assessment of healing using histopatologic approach, because the more variables that can be observed and described the changes that occur over morphofunctional changes, not only limited to the morphoanatomical changes.
