Abstract

Head injury is the leading cause of mortality in trauma patients. Besides, the numerous post traumatic morbidity sequele is still being a main problem in the field of brain trauma. In UK, 1500 per 100.000 of the population attend Accident and Emergency Department with head injury, while 300 per 100.000 per year are admitted to hospital, 9 per 100.000 per year die from head injury (Xiong Y, et al 2009). A good understanding of the patophysiology of the traumatic brain injury will lead to an optimal management of which. The management and therapy should be aimed to control the increase of intracranial preassure, maintain optimal cerebral perfusion by optimal blood supply and oxygenation delivery. Either surgical or nonsurgical management might be the best management, depends on the case. Careful decision must be taken in order to show good outcome. The clinical judgement of patient with a ‘borderline’ intracranial lesion is merely based on the attending physyician wheter to take a surgery or non-surgical management. However, the effectiveness of both treatments is still questionable. There are still lack of data regarding the functional and neurological outcome of each surgery and non-surgery (pharmacology) approach. Recent trend on medical treatment shows us a tremendous improvement of pharmacological intervention of brain injury patient.
