Abstract

Heart failure is a complex clinical syndrom that results from any structural or functional impairment of ventricular filling or ejection blood. Common risk factors for heart failure include cardiovascular disease (CVD), hypertension and cardiomyopathy. Diabetes mellitus is a chronic disease that causes CVD and heart failure is a major cardiovascular complication. Appropriate therapy for patients with heart failure and diabetes mellitus is necessary and one of therapy is beta blocker (bisoprolol). The aims of this study was to examine the profile of bisoprolol used and the possibility drug related problem (DRP) such as potential side effect and drug interactions in patients with heart failure and diabetes mellitus. It was a retrospective study during 1 January to 31 December 2017 at hospitalized patient of Cardiology Departement of RSUD Dr. Soetomo Surabaya. The results showed that from 30 patients who met the inclusion criteria, men were about 20 (67%) and women are about 10 (33%) with an age range 51-58 years (33,3%). The highest level of heart failure (according to NYHA) is level IV. Dosage of bisoprolol given was 1x1.25 mg (53.12%), 2x2,5 mg (40.62%), 1x2.5 mg (3.13%) and 1x5 mg (3.13%). Drug related problem (DRP) that found in this study were adverse drug reaction like hypotension and bradycardia (7%) and the risk of masking hypoglicemic symptoms (7%). Drug utilization of Bisoprolol in patient with heart failure at Dr. Soetomo Hospital were appropriate according to Diagnose and Therapy Guideline of Heart Failure.
