Abstract

Background: Improper endodontic treatment may cause defect at bifurcation area. Tooth with defect at the bifurcation area needs root canal treatment and endodontic surgery such as bicuspidization to repair the defect. Purpose: The aim of this case report is to report the treatment in mandibular right first molar with bifurcation perforation by bicuspidization surgery. Case: A 51 year-old female was referred to Conservative Dentistry Clinic Airlangga University, who suffered painful symptom in her mandibular right first molar. Intra oral examination showed an occlusal cavity with opened pulp, there is hole on the bottom of pulp chamber. There was no response to vitality test, percussion, palpation, and tooth mobility. Preoperative radiograph indicated the presence of radiolucency. Case management: The artificial wall and the closure of bifurcation perforation with glass ionomer cement were done. The biomechanical preparation was performed using crown down technique with ProTaper Next rotary files. The root canals were obturated using single cone technique, followed by bicuspidization procedure by making a vertical cut through the crown into the furcation. The root canal treated tooth was restored using post and porcelain fused to metal crown. Conclusion: The case report showed that endodontic treatment on molar with perforation on bifurcation could be done successfully.
