Abstract

The aim of this study was to identify the profile of fluid therapy in liver cirrhosis patient with complication of upper gastrointestinal bleeding at IRNA interna Dr Soetomo Teaching Hospital Surabaya due to the fluid type, dose, infusion rate, and duration of administration. It is a prospective study, with time limit sampling method and descriptively analyzed. The result showed that from 37 patients, type of fluid that used as bleeding resuscitation therapy at early hospital admission, rebleeding, and maintenance were normal salin, dextrose 10%, dextrose 5%, ringer lactate, ringer acetate, or HES 6%. Blood products which used were whole blood, packed red cells, thrombocyte concentrates, or fresh frozen plasma. Dose, transfusion rate, total and duration of administration of fluid therapy and blood product was depend on patient condition. Clinical data which used as the reference of fluid therapy are blood pressure, heart rate, respiratory rate, and urine production meanwhile, laboratory data which used as the reference of blood product therapy are hemoglobin level, platelets counts, or APTT. Drug related problem (DRP) identified in the fluid therapy which one patient could had more than one DRP. The type of DRP that identified were, improper fluid type selection (37,83%), improper blood product type selection (5,41%) improper fluid dose (40.54%), improper blood product dose (32,43%) unnecessary fluid therapy (83,79%) unnecessary blood product therapy (18,92%), indication that not getting the blood product therapy (27,03%).
