Abstract
Latar belakang: Penyakit diabetes melitus tipe 2 (DMT2) meningkatkan morbiditas dan mortalitas penyakit kardiovaskular termasuk komplikasi aterosklerotis. Patogenesis aterosklerosis pada DMT2 terutama disebabkan perubahan profil lipid dan lipoprotein. Kadar Lipoprotein(a)/Lp(a) yang tinggi diketahui merupakan faktor risiko terjadinya aterosklerosis. namun hubungan antara kadar Lp(a) dengan kekakuan arteri belum banyak diketahui. Tujuan: Menentukan hubungan antara kadar Lp(a) dengan kekakuan arteri yang diukur dengan baPWV pada pasien DMT2 di Poliklinik Endokrin Metabolik dan Diabetes RSUD Dr. Soetomo Surabaya. Metode: Penelitian analitik observasional cross-sectional ini dilakukan pada pasien DMT2 usia ≥45th di Poliklinik Endokrin Metabolik dan Diabetes RSUD Dr. Soetomo mulai Juni 2015-Agustus 2015. DMT2 ditentukan berdasar kriteria American Diabetes Association (ADA) 2014. Pemeriksaan Lp(a) menggunakan pemeriksaan aglutinasi Latex dan kekakuan arteri diukur dengan brachial ankle- Pulse Wave Velocity (baPWV). Analisis statistik hubungan kadar Lp(a) dengan kekakuan arteri dilakukan dengan uji non parametrik Spearman. Hasil: Jumlah sampel 39 pasien DMT2 dengan rerata usia 59,26±8.03 tahun, terdiri dari 23 pasien perempuan dan 16 pasien laki-laki. Terdapat 25.6% pasien dengan kadar Lp(a) ≥ 30 mg/dL dengan rerata kadar Lp(a) 21.66 ± 18.67 mg/dL. Terdapat 37 pasien (94.9%) yang mengalami kekakuan arteri dengan rerata baPWV 16.61 ± 2.57 cm/s. Hasil korelasi kadar Lp(a) dan baPWV menunjukkan p=0.88, r=0.026. Kesimpulan: Penelitian ini belum dapat menunjukkan adanya hubungan antara kadar Lp(a) dengan kekakuan arteri pada pasien DMT2, mungkin disebabkan oleh karena sebagian besar subjek penelitian (94.87%) telah mengalami kekakuan arteri dan ada faktor-faktor perancu lain yang belum dipertimbangkan, sehingga masih diperlukan penelitian lebih lanjut.
